To be in compliance with State and Federal Laws, Acument employees are required to wear Safety Glasses. All examination fees will be the responsibility of the employee.
Instructions:

Print the Order Form, Employee Letter, and Provider/Dispenser Letter.
Bring these documents to your listed provider/Dispenser when you order prescription safety glasses.
